Personal Documents
- Completed visa application form and two passport sized photographs with white background Matt Paper 35/45
- Original and copy passport with at least 6 months validity upon intended date of arrival in Indonesia and has 2 blank pages
- Bank Statement with minimum of USD 2000 (2 Lac BDT) and original and copy bank cheque
- Ticket booking (confirm ticket advisable)
- Hotel booking / information of accommodation (confirm accommodation advisable)
- Travel insurance
Professional Documents
- Letter from the applicant mentioning the purpose of the visit and intended destination (for Tourist)
- Reference/recommendation/NOC letter from the institution/company
- Copy of Trade license for business owner
Additional Documents
- Invitation/guarantee letter from sponsor in Indonesia (for Business, Conference, Social-Culture, Family)
- Copy of the Marriage Certificate or a proof of identity for those who are accompanying their spouses
- Statement from the applicants that they are aware of the place/destination of the visit in Indonesia (if applying through travel agent)
- Letter of authorization/statement from applicant to agent
- Letter of mission/assignment from the head/managing director of the agent to the employee who submits the documents
- ID of the employee who submits the documents (National ID and Company ID)
